  We intend to protect your privacy to the fullest extent, and are required by law to do so. We are permitted to make uses and disclosures of your health information, and for some we need your consent.  Some uses include treatment, payment and healthcare operations.  Certain uses or disclosures can be or must be made without your written consent, including public health issues (like communicable diseases), abuse or neglect, government agencies doing investigations, etc.

Your Individual Rights include requesting restrictions on certain uses or disclosures of your protected information; you must request this in writing with specifics.  We also will make reasonable attempts to communicate confidential information to you (for example, we do not leave test results
 on message machines or voice mail, but ask that you call us.)  You have the right to inspect and copy information contained in your "designated record set."  You have the right to ask us to account for our disclosures of your protected information.

Many of these provisions were a part of the HIPAA Law.
They protect your information, but sometimes this makes reporting information to you difficult, due to our need to protect your privacy.
