Bring your INSURANCE Card each visit, please.

COPAYS are due on the date of service.

Unless you tell us, we will bill your insurance company
 or health plan.  We will bill you for the balance due on your accounts once all payments and adjustments have been made.  Sometimes this is several months after your visit to the clinic, so please be patient.
 

If you do not have medical coverage of any kind, you will be personally responsible for the care provided.  Arrangements for payments may be
 made with our Financial Services staff.
